Availability of Application forms for FMS:

The forms for FMS will be available in designated branches of State Bank of India. Candidates can also download the form online through the website www.fms.edu. Candidates can also apply online through the same website and payment can be done through demand draft/credit card. The candidates that are interested in undertaking the application form through post have to clearly mention the registration number over the top of the requisite letter.
Otherwise, the candidates have to submit a copy of attested from duly signed by the Gazette officer. The candidates also have to submit a copy of Mark Sheet clearly mentioning the marks obtained by the respective candidate. Furthermore, the candidate should also clearly provide a signature sample along with two color photographs that should not be less than six months old. The photo should also mention the date of the day on which the photo was taken.
